The Guiding an Improved Dementia Experience (GUIDE) Model is an innovative initiative by the Centers for Medicare & Medicaid Services (CMS) aimed at enhancing care for individuals living with dementia and providing support to their unpaid caregivers. Launched on July 1, 2024, the GUIDE Model is set to run for eight years, involving 390 participating organizations across the United States.

Key Components of the GUIDE Model:

  • Comprehensive Dementia Care: The model emphasizes coordinated, person-centered care tailored to the unique needs of individuals with dementia. This includes comprehensive assessments, personalized care plans, and continuous care management to address both medical and psychosocial aspects of dementia.
  • Centers for Medicare & Medicaid Services
  • Support for Caregivers: Recognizing the vital role of unpaid caregivers, GUIDE provides them with access to evidence-based education, training programs on best practices for dementia care, and support services. Additionally, the model facilitates access to respite services, allowing caregivers to take temporary breaks from their responsibilities.
  • Care Navigation: Participants in the GUIDE Model are assigned a Care Navigator who assists them in accessing both GUIDE services and other necessary supports, including clinical services and non-clinical resources such as meals and transportation through community-based organizations.
  • Alternative Payment Methodology: CMS is testing a new payment system for providers delivering these comprehensive dementia care services, aiming to improve care quality while managing costs effectively.
  • Benefits of the GUIDE Model:
  • Improved Quality of Life: By offering personalized and coordinated care, the model seeks to enhance the overall well-being of individuals with dementia.
  • Caregiver Support: Providing education, training, and respite services helps reduce caregiver strain and improves their capacity to care for their loved ones.
  • Community Integration: The model aims to enable individuals with dementia to remain in their homes and communities longer, delaying or preventing the need for institutional care.

Clarke Lindsay in Urbana Illinois has day care

Lorrie,

I live at CCRC Medford Leas in Medford New Jersey. We have a respite room for independent living (IL) residents who are considering assisted living (AL). We definitely do not have daycare foIL residents. Condition in our assisted living have changed and are encouraging residents not to use AL since it is better to stay in place with a caregiver.

What is the CMS guide model?

Guiding an Improved Dementia Experience (GUIDE) Model | CMS The Guiding an Improved Dementia Experience (GUIDE) Model is a voluntary nationwide model test that aims to support people with dementia and their unpaid caregivers.

Model Overview

The GUIDE Model focuses on comprehensive, coordinated dementia care and aims to improve quality of life for people with dementia, reduce strain on their unpaid caregivers, and enable people with dementia to remain in their homes and communities. It will achieve these goals through Medicare payments for a comprehensive package of care coordination and care management, caregiver education and support, and respite services.

Our CCRC has a memory care unit, but residents who develop dementia often remain in independent living, mostly with a spouse as a 24/7 caregiver. There is discussion about how to support these residents, both the one with dementia and the caregiver.  What kinds of support, if any, does your community offer in such situations?  For instance, is respite care available for the person with dementia or for the caregiver? Is there a Caregivers Support Group?

Are you aware of any CCRC that offers on-site adult daycare?
