Do any ccrcs use a survey form for residents who have been in the healthcare unit(s) upon discharge? If so, would you be willing to share it with us? We are eager to develop one but see no need to reinvent the wheel if there's one out there.

All people discharged from our skilled nursing and assisted living are asked to respond to a list of statements. To make it quick and easy to respond, people are asked to rate each statement Unsatisfactory, Satisfactory, or Excellent. Comments can be added.

1: The room was clean and in good order.

2: The area around my room was quiet at night.

3: I was satisfied with the overall quality of the dining experience.

4: The nursing staff were responsive to my needs.

5: Team members listened to and resolved my concerns.

6: My pain was managed effectively.

7: The healthcare team kept me informed about my progress.

8: I have confidence in the health care provided.
