We are a large community with 12 apartment buildings. We have 6 social workers or resident care coordinators (some not licensed as SW in MD) assigned to apartments, another at the nursing and the assisted living buildings. There is also a social worker in the medical center. They help with transition primarily. Residents can connect them if they see another residents needing more help, family members work with the social workers.

We had a licensed clinical social worker in IL to assist with transitions of care, end of life issues, etc., who much to our consternation is leaving with reluctance shortly after a new CEO started work. Can you please tell me if your community provides a social worker and if so the services offered. Thank you.
